Step 3: Fill out the Business Application

You must complete a Business Application and attach the supporting documents listed below. Note: No fees will be charged for anyone becoming certified in 2006

Supporting Documents

  • [  ]   IPM Protocol for 1 pest your company manages. You must use the "EcoWise IPM Protocol Form".

  • [  ]   A signed copy of the "EcoWise Certified IPM Guiding Principles"

  • [  ]   Examples of your company's current inspection, monitoring and treatment record forms

  • [  ]   Examples of fact sheets, door hangers, or other written material (if any) used to provide information to your customers about IPM, sanitation, pest exclusion, habitat modification, or other issues

  • [  ]   Examples of brochures or other written information for your customers explaining your business or the services you offer (if any)

Step 1: Determine Eligibility
Step 2: Attend orientation session
Step 3: Fill out the Business Application
Step 4: Send in the Business Application for review
Step 5: Have at least 1 staff member become an EcoWise Certified IPM Practitioner
Step 6: Document at least 20 IPM service visits for 7 different customers following the Structural IPM Standards
Step 7: Schedule an office visit and field evaluation for your business
Step 8: Become fully certified
Step 9: Maintain your business and practitioner certifications
